Criteria For Curamus Assistance Application

  1. Only active members will be considered.
  2. All required information must be submitted prior to consideration.
  3. All normal avenues for assistance must have been exhausted.
  4. Life sustaining support will be given preference.
  5. Assistance will NOT be considered if the need was caused through negligence by the member or his or her guardian.
  6. Personal loans will NOT be considered.
  7. The applications for the following assistance will be considered:
    1. Mobility and other assistive devices; – two quotations
    2. Social; – certified social worker’s report
    3. Prevention of the aggravation of the disability; – doctor’s report
    4. Sport/recreational.
  8. Requests must be fully motivated and accompanied by the following documents:
    1. Official application form;
    2. Statement of income and expenditure;
    3. Copies of the last three month’s bank statement;
    4. Two quotations for items that must be purchased;
    5. Doctor’s or social worker’s report (as applicable);
    6. Invitation and/or motivation by chairperson of sport club (as applicable)
